Governance Groups and Communities of Practice for Foundational Responsibilities
Transforming the Public Health System in Minnesota
Local, state, and Tribal public health staff, and locally-elected officials, work together to decide how to implement foundational public health responsibilities in governance groups, and learn promising and best practices from each other in communities of practice.

- Communities of practice: In general, communities of practice are open to any local public health staff interested in joining, and often also to state and Tribal public health staff. Members may join any or all meetings as they're able.
- Governance groups: Governance group membership often requires being assigned by leadership or volunteering for a term over one or more years. Members consist of local, state, and Tribal public health leaders and locally-elected officials.

Foundational Public Health Responsibilities (FPHR) Community of Practice
Public health leaders and practitioners meet online monthly to learn about, discuss, and resolve questions related to the elements of the FPHR framework, its organization, definitions and activities, incorporating FPHR into current work, and more.

Assessment and surveillance (capability)
CHA-CHIP Community of Practice
This group meets regularly online, to discuss issues around community health assessment (CHA) and community health improvement planning (CHIP).

Regional Data Practice Groups
These groups provide a forum where Tribal and local public health can share lessons learned, seek advice, and coordinate efforts related to data collection, analysis, dissemination, reporting, and evaluation. They also provide opportunities to exchange health data information, and pool resources to develop capacity among participants.

Communications (capability)
MDH-LPH Communications Community of Practice
Communications leads and staff interested in communications from state, local, and Tribal public health meet every other month online, to discuss communications best practices..

This community of practice meets regularly to discuss health equity and community partnerships best practices.

SCHSAC Foundational Public Health Responsibilities (FPHR) Workgroup
This group meets online monthly, to develop recommendations for minimum standards that the public health system can use to measure if and how foundational responsibilities are implemented at the local and state levels. This group was recommended to convene by the previous SCHSAC Foundational Funding Workgroup (2023-2024). The SCHSAC FPHR Workgroup will deliver its recommendations to SCHSAC in December 2025.

Accountability and performance management (capability)
SCHSAC Performance Measurement Workgroup
This workgroup meets online monthly, and leads efforts to measure and assess the performance of Minnesota’s governmental public health system and its capacity to carry out public health responsibilities.

Emergency preparedness and response (capability)
Public Health Emergency Preparedness (PHEP) Oversight Group
This standing committee partners with MDH to guide strategic direction for public health emergency preparedness and response in Minnesota.

Communicable disease control (area)
Infectious Disease Continuous Improvement Board (IDCIB)
The IDCIB works to ensure an effective and efficient state-local partnership in the infectious disease area of public health responsibility. The group serves as a forum for regular communication, identification of issues, and joint problem-solving.

This group works broadly to monitor and advance state-local work in environmental health.
